Covid-19 Self Screening Appendix 1
Before you volunteer-- Please remember COVID-19 Self-Screening. Three questions about COVID-19 Status, Symptoms, and Exposure, based on recommendations from the CDC and the NMDOH are asked with the goal of preventing infection in the workplace.
Volunteers are not required to submit their answers to the Department of Cultural Affairs.
